Note: Please complete our office forms and bring them with you on your initial visit.  You can locate the forms by visiting our website: http://www.bodyprochiropractic.com and click on the "forms" tab. In addition, please bring any pertinent xrays/mri's. If you do not have xrays, please contact us at :  [email protected]   and we will order what is necessary PRIOR to your first visit. Having these will allow the Doctor to gain a better understanding of your body and will also allow care to begin on the first day.
